Our focus is senior care guidance, including assisted living, memory care, in-home care referrals, and family consultation.

Heather Smith, Co-Founder
Heather Smith is the heart behind Summit Care Navigation. With a background in social work and family support, Heather understands how overwhelming care decisions can feel when families are unsure where to begin.
Her approach is compassionate, practical, and personal. She helps families slow the process down, understand their options, and feel supported through each step. Whether a family is exploring assisted living, memory care, in-home support, or other care resources, Heather’s goal is to provide clear guidance and trusted local support.
Heather is a certified Community Health Worker with years of experience supporting seniors, families, and individuals in need of care resources. Her background in care coordination, education, advocacy, and community-based support gives her a practical understanding of the emotional and logistical challenges families face when care needs change.
Glenn Smith, Co-Founder
Glenn Smith brings decades of business, leadership, project management, and problem-solving experience to Summit Care Navigation. As co-founder, Glenn supports the company’s operations, strategic planning, marketing, systems development, and long-term growth.
His background includes owning and managing businesses, leading complex projects, mentoring individuals through challenging life transitions, and developing practical systems that help people move from uncertainty to action. Glenn’s role with Summit is to help build a reliable, organized, and scalable company that serves families well today and can grow responsibly in the future.
Together, Heather and Glenn are building Summit Care Navigation as a trusted local resource for families who need guidance, clarity, and compassionate support during important care decisions.
